Moffitt Cancer Center: Partnering with Philips to improve efficiency in radiation therapy

Introduction

Who/Where

The H. Lee Moffitt Cancer Center & Research Institute is one of the nation's top cancer facilities. Located in Tampa, Florida, it is the only Florida-based cancer center with the National Cancer Institute (NCI) designation as a Comprehensive Cancer Center for excellence in research, clinical trials, prevention, and cancer control. Moffitt is also a member of the National Comprehensive Cancer Network, a prestigious alliance of the country's leading cancer centers.

 

Challenge

Moffitt faced major space constraints to increase radiation treatment capacity and needed a solution to keep up with rapidly growing demand. The Center was looking for an advanced treatment planning system that offered seamless connectivity to their existing and new VMAT capable linear accelerators, scalability to grow with the center's needs and the flexibility to integrate the latest cutting edge technology.

 

Solution

The Philips Pinnacle3 radiation treatment planning system ranked highest in efficiency, scalability and was the dosimetrists' preference due to ease of use. Pinnacle3 offers the flexibility needed to support the center's expansion in addition to being customizable to Moffitt's specific demands and growth needs, providing planning functionality and ease of learning.

As part of the Center's expansion, Moffitt recently purchased Varian Linear Accelerators (Linacs) capable of volumetric modulated arc therapy (VMAT) delivery, and they were on the search for an advanced radiation therapy planning system offering scalability and flexibility. Dosimetry and physics staffs were involved in a rigorous analysis of top systems. Systems were reviewed based on pre-determined criteria, including ease of use, ability to handle 4D treatment planning, number of image sets simultaneously registered, clinical ability, and configuration.

Challenges

Moffitt faced several challenges in radiation therapy planning, including:

 

 Scalability: Moffitt is on a steep growth curve, and the Center faces major space constraints as it continues to expand and integrate. The ability to template into other satellite centers without degradation in data quality and speed of transmission was a high priority.

 

 Workflow Efficiency: Treating more than 170 patients a day, Moffitt needed an advanced system to help increase productivity, provide seamless integration, and keep up with high demand. With a large faculty with frequent travel requirements, they needed to provide rapid remote plan review and peer review.

 

 Flexibility: As a top cancer center, Moffitt needed to stay on the cutting edge of new technologies. The basic promise of volumetric modulated arc therapy (VMAT) delivery is to cut the treatment time per patient roughly in half, allowing the facility to increase treatment capacity and productivity. Moffitt needed a workstation to work seamlessly with their VMAT capable Linear Accelerators.

 

 Advanced Treatment Planning: 4D treatment planning was essential for the Moffitt team to remain on the cutting edge. With imaging expansion into 4D PET, they also needed to manage dozens of image sets per patient. With its previous system, Moffitt also ran into problems moving structure image sets between environments, and difficulties with image registration and the size of the data sets.

The solution: Philips Pinnacle³

Moffitt chose Philips Pinnacle3 radiation therapy planning system for hospital-wide use, to work with its Varian Linacs rather than selecting Varian's bundle option. Pinnacle3 ranked highest in efficiency, scalability, and was the dosimetrists' preference, primarily due to ease of use. In addition to highly acclaimed physics, the Pinnacle3 system offers the flexibility to support VMAT delivery when Moffitt is ready to move to this type of therapy.

The Philips SmartEnterprise solution is a new hardware system with full scalability that allows up to 16 Pinnacle3 simultaneous users per computing module and the flexibility of virtually limitless expansion by adding modules, clients and user licenses.

"Philips solution offers superior planning functionality, accuracy and ease of learning, and it will allow Moffitt to deliver high-quality treatment plans more efficiently. With the auto contouring tools, our dosimetrists do not have to spend time contouring by hand. They will be able to perform simple plans faster and more complex plans can have more iterations performed, resulting in quicker, more accurate treatment."

 

Philips Pinnacle3 supports Moffitt's expansion through unique open architecture that can create a centralized database that eliminates the need to transfer files. This streamlines workflow to offer a scalable, cost effective multi-center solution. The Pinnacle3 system provides planning capabilities at multiple locations and the flexibility of remote users.

 

With Moffitt's previous vendor, the number of simultaneous users was dependent upon the number of workstations (nine in total); now, with the addition of the Pinnacle3 SmartEnterprise system, Moffitt has the infrastructure in place to add more users in a simple and cost effective way as demand grows.

 

"Philips also offers highly skilled professional support, alleviating a hefty burden on our internal IT staff," said Mark Russell, manager, Dosimetry Services for Moffitt. "System administration management is minimal with the benefit of a highly functional system that templates into our enterprise environment. We anticipate that this system architecture, along with having Philips provide the support, will lower costs by as much as 25 percent."

Installation took place as planned in December 2008 with commissioning in early 2009. "Philips has stayed to their timeline and we got what we paid for-a comprehensive planning system that will allow us to deliver high-quality treatment plans faster and offer time and cost savings to our staff and facility," said Dr. Stevens.
